Screening may have saved my life. PSA had been <2 for several years but then jumped from 2 to 4.5 in 12 months. Typically not an actionable level but the rate of change concerned my internal medicine doc so he recommended I see a urologist. The urologist agreed with his assessment and recommended performing a biopsy to ensure it wasn’t cancer (he didn’t think it was). Surprise! 2 out of 10 biopsy needles showed cancer was present (Gleeson score of 5-6 if I remember correctly). We decided to be aggressive and had a prostatectomy. The anterior surface of the gland was totally covered in cancer. Apparently, the biopsy needles weren’t quite long enough to reach the anterior surface except for the 2 samples that were positive. With the anterior surface being totally covered, it would have likely metastasized to my lymph nodes fairly soon had I not had treatment when I did. The fact I had a good history of screening really helped in quickly identifying that something was abnormal.
